The mission of Hospice of the Sacred Heart is to provide comfort, care, hope and choice to patients and their families while guiding them through their end of life journey.


COMFORT

Take comfort from our emotional and spiritual support. Hospice of the Sacred Heart strives to enable patients to spend their last days with dignity and quality, surrounded by the ones they love. We treat the person rather than the illness, focusing on quality rather than length of life. We foster a family-centered approach in making decisions. Our chaplains provide individualized, non-denominational support to patients and families, often serving as a liaison with their religious community.

CARE

We care for the physical needs of each patient. Hospice of the Sacred Heart is committed to alleviating distressing symptoms and minimizing pain, while keeping the patient in control. Guided by our interdisciplinary plan of care, our staff is available 24 hours a day, seven days a week, usually in the patient’s home—with family members and friends serving as the primary, hands-on caregivers. However, hospice services are also available to patients in nursing homes and assisted living facilities. Hospice nurses educate family members to care for the patient properly and safely. During the time the patient is under hospice care, our nurses keep the primary physician, our Medical Director and the family informed about the patient’s condition.

HOPE

Each patient’s emotional well-being depends on hope… hope that pain will be controlled… hope for quality time with family and assistance for caregivers… hope that he or she won’t die alone. Hospice of the Sacred Heart offers compassion and companionship to our patients. Our staff of trained volunteers provides the caregivers with relief from the sometimes overwhelming task of caring around the clock for the one they love. After the patient has died, we offer his or her family members continued bereavement support. Hospice of the Sacred Heart restores your hope.

CHOICE

Our social services give you real choice. Hospice of the Sacred Heart offers assistance with both practical and financial concerns. Our social workers help with advance directives and evaluate the family’s need for volunteers and other support services. We also facilitate communication between the family and many community resources.
